Installation Form Tips

A properly completed Streetlighting Installation/Change Request Form will help ensure a quick and accurate response to your request. Please consider the following tips when completing your form.

*It is not necessary to list pole numbers for company owned lights that have not yet been installed.

Pole # - The six-digit pole number from the pole plate (if you have it). This number is found on the license plate of the pole.

Example:

Signature - The signature should come from lighting customer (government entity) not a developer or contractor.

Return Fax # - The return fax number will be used to fax the completed form back to you with an assigned request number.
